Sign-ups for Healthy U Delmarva are currently being held. To sign-up, please call the Healthy U office to schedule an appointment, or sign-up at one of the following sites:
- Healthy U Office:
103 Power Street, Salisbury MD - Curves of Salisbury East, Fruitland, Laurel, Berlin/Ocean Pines, Princess Anne and Crisfield
- Lower Shore YMCA (Pocomoke)
- Wicomico County Health Department
If you were a Healthy U member in the past and would like to continue to be one, you must sign-up again.
Measurement Form
Healthy U members have the option of signing up for Healthy U using the
sign-up sheet above or by using the measurement form below. The measurement form
allows members to obtain their weight and waist-to-hip ratio, which allows for
an initial assessment of health risks. Members are then able to come in
throughout the year for updated measurements to assess their progress.
To get measured you may contact the Healthy U office to schedule a time to complete the form below.

Once you become a member using either the sign-up sheet or the measurement form, you are eligible for your new membership card. Membership cards allows members to enjoy discounts at area locations. For more information regarding the discount program click the link below.
